Patient Rights
As a patient at Lower Umpqua Hospital, you will be treated with dignity.
We believe you have the right to:
Considerate and respectful care.
You should expect consideration of your medical, social, spiritual and
emotional well-being. Your race, religion, cultural values, sex, disability,
age, sexual preference or ability to pay will not affect your care.
Information about your condition and treatment.
You should expect your healthcare team to provide you a clear, concise
and understandable description of your condition, treatment choices and
the benefits and risks of each choice. Your nurse will be happy to examine
your chart with you.
Participation in decisions about your care.
You have the right to make decisions regarding your medical care, up to
and including Advance Directives (i.e., Living Will). You also have the
right to refuse a diagnostic or therapeutic procedure to treatment. If
you elect to refuse the treatment, you will be informed of the medical
consequences of your decision.
Privacy and confidentiality.
Case discussions, examinations, consultations and treatment are confidential
and will be conducted as such, The confidentiality of your medical record
will be safeguarded.
