Job Summary

The Staff Pharmacist is responsible and accountable for the provision of a broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy. Through various assignments within the department, this pharmacist provides support for inpatient and outpatient services and delivers optimal medication therapy to patients with a broad range of disease states. The Staff Pharmacist works in collaboration with multidisciplinary care providers to monitor, assess, and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.

This position reports to the Director of Pharmacy Services and does not have direct reports.

Essential Duties

  • Perform all job duties and functions within the scope of practice for a Registered Pharmacist, as defined by the Oregon State Board of Pharmacy.
  • Practice in the assigned service and care area and/or rotate through different service area assignments.
  • Provide oversight and ongoing supervision for all activities performed by supportive personnel, including pharmacy technicians, clerks, and interns, to ensure compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.
  • Perform pharmacist tasks including compounding, drug therapy reviews, verification, patient counseling and education, and medication therapy management. Review, interpret, and accurately dispense prescribed medications.
  • Apply evidence-based medicine, national guidelines, and hospital policies and procedures when creating recommendations to health care providers for optimizing patients’ medication regimens.
  • Analyze and review medication lists for potential patient safety problems, including drug interactions, suboptimal medication regimens, and other concerns.
  • Research, develop, and implement drug utilization and disease state management strategies and intervention techniques to deliver high-quality, cost-effective healthcare.
  • Comply with policies and procedures related to the preparation and supervision of all sterile and non-sterile compounding.
  • Remain accountable for safe medication storage, diversion monitoring, and other key pharmacy inventory activities.
  • Document all clinical activities and interventions accurately and completely.
  • Participate in education and training of healthcare practitioners, including students, interns, nurses, technicians, and other healthcare professionals.
  • Participate in quality initiatives to improve services by monitoring processes, documenting, and analyzing data.
  • Serve as a drug information resource to patients, providers, and other members of the healthcare team.
  • Demonstrate knowledge of all current federal, state, and local laws and regulations pertaining to the practice of pharmacy.
  • Demonstrate knowledge of and ability to assess patient needs, incorporating age- and disease-specific characteristics to optimize pharmacotherapy.
  • Deliver customer service and/or patient care in a manner that promotes goodwill and is timely, efficient, and accurate.
  • Demonstrate medical literature evaluation skills and an understanding of evidence-based medicine.
  • Maintain and enhance current knowledge and skills related to pharmacy and healthcare by reading pharmacy-related journals, participating in educational opportunities, maintaining professional networks, and participating in professional organizations.
  • Maintain awareness of developments in pharmacy care and pursue best practices that enhance performance and health outcomes. Stay up to date on state and federal law and policy changes.
  • Demonstrate proficiency in hospital computer systems and software programs and applicable office equipment.
  • Interact professionally and communicate clearly and effectively, both verbally and in writing, with members of the pharmacy, other health care professionals, and the public.
  • Maintain confidentiality and think and act independently with minimal supervision.
  • Organize and prioritize workload effectively, handle multiple tasks, and make critical decisions to ensure pharmacy services are provided in a timely manner.
  • Maintain regular, consistent, and punctual attendance at the assigned job location.
  • Participate in department and staff meetings and committees.
  • Perform other duties as required or assigned.

Qualification Requirements

Education and License

  • Graduation from an accredited College of Pharmacy with a Bachelor of Science in Pharmacy degree or Doctor of Pharmacy (Pharm.D.) degree required.
  • Current Oregon Pharmacist license required.
  • American Heart Association Basic Life Support (BLS) Certification required and must be obtained within 60 days of hire.

Certifications

  • Relevant certification through the Board of Pharmacy Specialties (BPS) preferred.

Experience

  • Minimum of one year of pharmacy practice experience preferred.
  • Completion of an ASHP-accredited residency preferred.

Additional Qualifications

  • Ability to commute to Reedsport, OR 97467 is required.
  • Ability to maintain confidentiality and work independently with minimal supervision.
  • Strong organizational skills and ability to prioritize workload effectively.
  • Ability to handle multiple tasks and make critical decisions in a timely manner.
  • Clear and effective verbal and written communication skills.
  • Proficiency with hospital computer systems, software programs, and applicable office equipment.

Work Environment

Works indoors in an air-conditioned office environment.

Physical Demands

Frequent use of computers, telephones, and standard office machines. Must be able to stand, sit, stoop, twist, kneel, reach, push, and pull using proper body mechanics, and move or lift varying amounts of weight up to a maximum of 50 lbs.
