Job Summary

The Staff Pharmacist is responsible and accountable for the provision of a broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy. Through various assignments within the department, this pharmacist provides support of inpatient and outpatient services and delivers optimal medication therapy to patients with a broad range of disease states.

The Staff Pharmacist works in collaboration with multidisciplinary care providers to monitor, assess and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.

Essential Duties

  • Performs all job duties and functions within the scope of practice for a Registered Pharmacist, as defined by the Oregon State Board of Pharmacy.
  • Practices in the assigned service and care area, and/or rotates through different service area assignments.
  • Provides oversight and ongoing supervision for activities performed by supportive personnel to ensure compliance with laws, regulations, and required records.
  • Performs pharmacist tasks including compounding, therapy reviews, verification, counseling/education, and medication therapy management; reviews, interprets, and dispenses prescribed medications.
  • Applies evidence-based medicine, national guidelines, and hospital policies when making recommendations to optimize medication regimens.
  • Analyzes medication lists for patient safety problems (interactions, suboptimal regimens, etc.).
  • Researches, develops, and implements drug utilization and disease state management strategies and interventions.
  • Complies with policies and procedures related to sterile and non-sterile compounding preparation and supervision.
  • Accountable for safe medication storage, diversion monitoring, and key inventory activities.
  • Documents clinical activities and interventions accurately and completely.
  • Participates in education and training of healthcare practitioners (students, interns, nurses, technicians, and others).
  • Participates in quality initiatives by monitoring processes, documenting, and analyzing data.
  • Serves as a drug information resource to patients, providers, and the healthcare team.
  • Maintains knowledge of applicable federal, state, and local pharmacy laws and regulations.
  • Assesses patient needs using age- and disease-specific characteristics to optimize pharmacotherapy.
  • Delivers customer service/patient care that promotes goodwill and is timely, efficient, and accurate.
  • Demonstrates medical literature evaluation skills and understanding of evidence-based medicine.
  • Maintains and enhances pharmacy/healthcare knowledge through journals, education, networks, and professional organizations; stays current on law/policy changes.
  • Demonstrates proficiency in hospital computer systems and software programs and applicable office equipment.
  • Communicates clearly and effectively (verbal and written) with the pharmacy, other healthcare professionals, and the public.
  • Maintains confidentiality; thinks and acts independently with minimal supervision.
  • Organizes and prioritizes workload, handles multiple tasks, and makes critical decisions to ensure timely pharmacy services.
  • Maintains regular, consistent, and punctual attendance at the assigned job location.
  • Participates in department/staff meetings and committees.
  • Performs other duties as required or assigned.

Qualification Requirements

Education and License

  • Graduation from an accredited College of Pharmacy with Bachelor of Science Pharmacy degree or Doctor of Pharmacy (Pharm.D.) degree (required).
  • American Heart Association Basic Life Support (BLS) Certification (required; must be obtained within 60 days of hire).
  • Current Oregon Pharmacist license (required).

Certifications

  • Relevant certification through Board of Pharmacy Specialties (BPS) (preferred).

Experience

  • Minimum of 1 year of pharmacy practice experience (preferred).
  • Completion of ASHP accredited residency (preferred).

Work Environment

Works indoors in an air-conditioned office environment.

Physical Demands

Frequent use of computer and phone, and standard office machines. Must be able to stand, sit, stoop, twist, kneel, reach, push, pull using proper body mechanics and move/lift varying amounts of weight to a maximum of 50 lbs.
